 Below are all the attachments needed with the exception of parking directions. •Group Registration Form- is for you to fill out. This gives us the estimated head count, shirt sizes, time slot choices, point persons and contact information. We would like to have that completed by Thursday, Oct. 15th •Individual Registration Form- is for each person going to fill out. We would like to have that completed by Friday, Oct 10th. •Release and Grant of Rights- is for each person to fill out regardless of age. We would like to have that completed by Friday, Oct. 9th. •Service-Learning Certification Form- is for each High School student to present to their Service Learning Coordinator at his or her school. •Volunteer Manifest- is for the contact person to fill out and keep with them on the trip. •Student Instructions -is the assignment High School students need to do in conjunction with the event to earn their service learning credit. We would like to have that completed by Dec. 7th. On the day of the event while in route please call in your actual numbers and shirt sizes to EJ @ 1 (213) 924-3147 We will have them bundle up for you upon your arrival. Please don't let your students off the bus until our meet and greet team gives them instructions and issues them the tees "The Solution" has been a life-changing experience for many students!! *Please return required forms so that we can reserve your preferred time. * |
